Acute pancreatitis and macrophage activation syndrome are rarely seen in systemic lupus erythematosus. We report the case of a young female patient without any noteworthy prior history, who was hospitalized for abdominal pain associated with acute pancreatitis possibly related to SLE with associated macrophage activation syndrome. This association of pancreatitis with macrophage activation syndrome and lupus, although reported in the literature, its inaugural nature of lupus disease remains exceptional. The search for other associated signs, particularly cutaneous and articular, can help avoid diagnostic errors.
