Portal vein calcifications are rare. They are often associated with portal hypertension. We report a case of a 75 year-old woman who presented pain in the right upper quadrant. Radiologic examination had found a portal thrombosis with calcification of the portal vein wall and a gallstone. The diagnosis of portal calcifications can be difficult and confusing with other pathologies. CT-scan is the examination of choice. Recognition of portal calcifications is important because they represent a major factor for operative mortality in liver transplantation.
